Please use website forms carefully
Website forms are for general questions, appointment requests, and similar administrative contact. Please do not include Social Security numbers, insurance ID numbers, detailed medical histories, clinical notes, injury photos, or other highly sensitive health or identity information in a general website form or ordinary email unless our staff asks you to use a method they designate. For clinical information, call (941) 488-0222 and ask how we prefer you to send it.

Information we collect
Information you give us. Depending on the form, you may provide your name, email, phone number, a message, preferred appointment date, time, and location, whether you are a new patient, and, if you choose, an uploaded New Patient Packet. You may also reach us by phone, mail, or email. Please keep free-text messages focused on contact and scheduling when you can.

Information collected automatically. When you visit, our website platform and related tools may collect technical information such as IP address, browser and device details, pages viewed, referring website, and approximate time of visit. Approximate location may be inferred from IP address.

Advertising tools. This site does not currently use Google Analytics, a Meta (Facebook) Pixel, or similar advertising pixels. It does include a link to our Facebook page. If we add analytics or advertising tools later, we will update this policy.

How we use information
We use website-related information to respond to messages; schedule or follow up on appointments; handle new-patient paperwork you choose to upload; send administrative or appointment-related communications you request; send practice emails if you subscribe (you may unsubscribe in the email or by contacting us); operate and improve the website; help prevent spam and automated abuse; show our office map; and meet legal requirements or protect patients, staff, and the practice.

Keeping information
We keep website form submissions, related communications, and technical logs as long as reasonably needed for the purposes above, for ordinary operational and legal recordkeeping, and as required by law. Medical-record information and other PHI are also subject to our medical-record practices and HIPAA requirements.

Sending information over the internet is not completely secure. Please avoid highly sensitive medical or identity information in general website forms unless our staff directs you to a specific method.

Children
This site is intended for adults seeking foot and ankle care information for themselves or family members. We do not knowingly collect personal information online from children under 13 for marketing. If you believe a child submitted information here, please contact us.
